The following forms should be submitted to the admissions office along with your application. These forms may be downloaded at the links below or requested from Bethany's admissions office.

• Academic Reference form
• Personal Reference form

Items to submit once admitted

Please submit:

• Enrollment Information Form
• Enrollment Contract
• Permission for Treatment, Dispensing Medications, and Travel Form—must be signed in the presence of a notary public
• Community Life Expectations Agreement
• Power of Attorney form
• Immunization records from applicant's physician or the health department.
• Proof of international health insurance
